I’m Pam and over my 27-year career as an occupational therapist, I have devoted myself to working with individuals so that they can live a life which is fulfilling. Through careful assessment, I can better understand the challenges being faced in order to come alongside, and help find effective solutions to maximise independence.

Following acquired and traumatic brain injury, I work with clients to understand their capabilities, the challenges faced and the impact this has on everyday life. I am committed to helping find effective solutions to overcome barriers, maximise independence in tasks that are important and above all, help live a fulfilling life.

About Me

My 27-year career as an occupational therapist has been devoted to working in neuro rehabilitation. Working in the NHS for 12 years enabled me to develop invaluable experience and an understanding of the rehabilitation journey for individuals following acquired and traumatic brain injury. Delivering community rehabilitation in the private sector for the next 15 years was a privilege, enabling clients to engage in the activities which were important to them at home, in the community and at work. This experience has enabled me to work in partnership with a wide range of other individuals including case managers, solicitors and insurers and allowed me to be creative in finding effective solutions to help clients achieve what matters most to them.I am highly empathic and strive to deliver the very best rehabilitation to my clients. I believe in the importance of spending time listening and understanding what is important to them. This approach, combined with a wealth of knowledge and a kind and honest ethos, enables me to help individuals maximise their independence, and above all, live a fulfilling and meaningful life.Read about the services I offer here.

Privacy Policy

Introduction
This statement provides an overview of the personal data that Pam Foreman Occupational Therapy Services collects and processes, how we do it and our purposes for doing so. We may also share your data with third parties and provide information about why we may need to do this. We explain how you may gain access to the data we hold about you and your rights in relation to your personal data.
Collection of personal information
Personal and sensitive information on clients is collected, stored and used for the purpose of providing occupational therapy services. This information may be collected by email, telephone call, post, forms and in person.
Most of the personal information we process is provided to us directly by the client and/or whoever refers the client to our service. With consent, information may also be collected from other professionals working with the client (for example, physiotherapist, speech and language therapist, psychologist, case manager). We may also collect information about family members where this relates to the client, for example, contact details for family.What type of information we might collect
As part of the assessment process and to deliver rehabilitation programmes, we may collect and hold the following personal information for our clients:

  • Name

  • Date of birth

  • Address

  • Telephone numbers

  • Next of kin and family details

  • Email address

  • Details of legal, deputyship, medical and rehabilitation team

  • Property information

  • Medical details

  • Manual handling requirements

  • Information from other healthcare professional reports

  • Employment details

  • Education details

  • Details of accident / injury

  • Physical or mental health conditions

  • Rehabilitation history

  • Proceedings relating to family / welfare matters

How we use data
We use the information to:

  • Plan and provide occupational therapy services appropriate for the client’s needs.

  • To communicate with client’s and referrers via email, telephone, mobile phone messages in relation to arranging appointments, general communication in between sessions, sending reports and other documents and copying into communication with other professionals involved with the rehabilitation programme.

  • Sending invoices.

  • Complete professional record keeping.

  • Record outcomes of assessment and treatment sessions.

Whenever possible, personal identifying information is removed from the information.We may need to share your information with relevant parties when necessary, for example with other professionals working with the client (for example, physiotherapist, speech and language therapist, psychologist, case manager).In some instances, it may be necessary to transfer your data outside the European Union. Where this is the case, we will seek further consent from you to do this and will take all necessary steps to safeguard your information and ensure your data protection rights are maintained.Meeting our professional obligations
It is a legal requirement for all occupational therapists to be registered with the Health and Care Professions Council (HCPC). The HCPC has clear standards of conduct, performance and ethics that registrants must adhere to. These standards affect the way in which information is processed and shared including:
Standard 2: Communicate appropriately and effectively
"You must share relevant information, where appropriate, with colleagues involved in the care, treatment or other services provided to a service user."
Standard 10: Keep records of your work
"You must keep full, clear, and accurate records for everyone you care for, treat, or provide other services to. You must complete all records promptly and as soon as possible after providing care, treatment or other services. You must keep records secure by protecting them from loss, damage or inappropriate access."
Why we process personal information
Under the General Data Protection Regulations (GDPR), the lawful basis we rely on for processing this information is one of ‘legitimate interest’. Occupational therapy services cannot be safely or adequately delivered without processing personal information. The main purpose therefore for processing client’s personal data is to enable us to pursue our legitimate interest in assessing and rehabilitating clients or for purposes which directly support this work.
Data relating to health of an individual is classified as ‘special category data”. The general data protection regulations specify that health professionals who are ‘legally bound to professional secrecy’ may have a lawful basis for processing this data. Occupational therapists are legally bound to keep client information confidential, and it is under this condition that we store and process personal information.How we store personal information
We are committed to ensuring that personal information is secure. To prevent unauthorised disclosure, access, destruction, or alteration we have secure electronic systems, robust processes and systems and procedures in place. All information is stored securely in our systems to ensure there is a complete record of the occupational therapy services provided. Our systems are compliant with general data protection regulations. Documents containing confidential information, for example, reports are individually password protected. Any paper based personal information such as assessments are stored securely in accordance with general data protection regulations.
The minimum amount of personal information will be taken out of the occupational therapist’s office base. When your information is taken out of the office base, it will be kept with the occupational therapist or locked securely in the boot of a car.How long we keep your data
We will not keep your data for any longer than is strictly necessary. We are obligated to store information on clients for a period of seven years after we cease to provide occupational therapy services to the client. For children we store their information for a period of seven years after their 18th birthday. Personal data relating to other contacts will be stored for a period of two years after our last contact with you unless otherwise required by law.
Legal rights
You have certain legal rights when it comes to your personal information, which include:

  • Your right of access - You have the right to ask us for copies of your personal information.

  • Your right to rectification - You have the right to ask us to rectify information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

  • Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.

  • our right to restriction of processing - You have the right to ask us to restrict the processing of your information in certain circumstances.

  • Your right to object to processing - You have the right to object to the processing of your personal data in certain circumstances.

  • Your right to data portability - You have the right to ask that we transfer the information you gave us to another organisation, or to you, in certain circumstances.

We have procedures in place to assist you, should you wish to exercise these rights, except in the case where it directly conflicts with the rights of others or our legal obligations. If you wish to speak with us about how your data is handled, please use the following contact:

Access to data
If you wish to request a copy of the information we hold about you, please apply in writing. The address will be given to you by the occupational therapist. You will need to satisfy Pam Foreman Occupational Therapy Services that the personal information that you are requesting belongs to you, or that you are the legal guardian of that person. A copy of records is provided free of charge. We are obliged to satisfy all such requests within one month of receiving the request or up to three months if we consider the request to be complex. You will be notified within the first month if we are not able to provide you with your personal data within this time and with the reason for the delay.
How to complain
If you have a concern about how we handle your data, you have the right to complain. You can do this by contacting us directly at pam@pamforeman.com.
